REAL PERSON INSURANCE AGENTS

 

For registration, you must make a pre-application at www.sigorta.org.tr before coming to our Chamber.

 

 

 

DOCUMENTS REQUIRED FOR REAL PERSON INSURANCE AGENTS:

 

1ST PETITION

 

2. COPY OF BIRTH CERTIFICATE OR COPY OF T.C. IDENTITY CARD

 

3.CERTIFICATE OF RESIDENCE (HOME ADDRESS)

 

4.ARCHIVE REGISTERED CRIMINAL RECORD CERTIFICATE (TO BE TAKEN FOR OFFICIAL INSTITUTION)

 

5. PHOTOCOPY OF SEGEM CERTIFICATE

 

6.COPY OF THE MOST RECENT GRADUATED SCHOOL DIPLOMA (PROVIDED THAT THE ORIGINAL IS SUBMITTED OR NOTARIZED COPY)

 

7. PHOTOCOPY OF TRADE REGISTRY GAZETTE

 

8. PHOTOCOPY OF TRADE REGISTRY CERTIFICATE

 

9. COPY OF ROOM REGISTRATION

 

10.ANNEX-7 DECLARATION OF ASSETS

 

11. FOREIGN LANGUAGE CERTIFICATE (IF FOREIGN LANGUAGE IS KNOWN)

 

12. COPY OF PROFESSIONAL LIABILITY INSURANCE (IF ISSUED)

 

13.(IF AN AGENCY CONTRACT HAS BEEN MADE WITH AN INSURANCE COMPANY) COPY OF THE CONTRACT MADE WITH THE INSURANCE COMPANY

 

14. STATEMENT CONTAINING AGENCY CONTACT INFORMATION

 

15. OFFICIAL DOCUMENT TO BE OBTAINED FROM THE STATIC IP SERVICE PROVIDER ISSUED IN THE NAME OF THE AGENCY

 

16. 2 INVOICES ISSUED IN THE NAME OF THE AGENCY (ELECTRICITY, TELEPHONE, NATURAL GAS, WATER, ETC.)

 

17. TWO PASSPORT SIZE PHOTOGRAPHS

 

18. DOCUMENTS RELATED TO PROFESSIONAL EXPERIENCE (IF ANY)

 

 

 

DOCUMENTS REQUIRED FOR TECHNICAL STAFF OF REAL PERSON INSURANCE AGENCIES:

 

 

 

1. COPY OF BIRTH CERTIFICATE OR COPY OF T.C. IDENTITY CARD

 

2.CERTIFICATE OF RESIDENCE (HOME ADDRESS)

 

3.ARCHIVE REGISTERED CRIMINAL RECORD CERTIFICATE (TO BE TAKEN FOR OFFICIAL INSTITUTION)

 

4. PHOTOCOPY OF SEGEM CERTIFICATE

 

5.COPY OF THE MOST RECENT GRADUATED SCHOOL DIPLOMA (PROVIDED THAT THE ORIGINAL IS SUBMITTED OR NOTARIZED COPY)

 

6. DOCUMENTS RELATED TO PROFESSIONAL EXPERIENCE (IF ANY). (IF NOT A 4-YEAR UNIVERSITY GRADUATE, SOCIAL SECURITY SERVICE BREAKDOWN AND MORE  

 

   SGK INSURED RESIGNATION DECLARATION FROM THE INSURANCE AGENCIES WHERE HE WORKED BEFORE)

 

7. PHOTOCOPY OF SEGEM CERTIFICATE

 

8. SGK EMPLOYMENT DECLARATION

 

9. TWO PASSPORT SIZE PHOTOGRAPHS/ GSM NO.

 

 

 

ALL DOCUMENTS, PHOTOGRAPHS AND DECLARATIONS MUST NOT BE OLDER THAN SIX (6) MONTHS. THE COPY OF THE DOCUMENTS TO BE SUBMITTED AS A COPY SHALL BE APPROVED BY THE COMPETENT AUTHORITIES OR THE ORIGINAL SHALL BE SUBMITTED SEPARATELY DURING THE DOCUMENT SUBMISSION.

PROFESSIONAL EXPERIENCE MUST BE PROVED WITH OFFICIAL DOCUMENTS SUCH AS TRADE REGISTRY RECORD, DOCUMENT TO BE OBTAINED FROM SGK.

INSURANCE AGENCIES MUST MAKE AN AGENCY CONTRACT WITH AT LEAST ONE INSURANCE COMPANY WITHIN 6 MONTHS AFTER REGISTRATION AND SUBMIT IT TO THE CHAMBER WHERE THE APPLICATION IS MADE.
